Bridges Reentry Awarded 2026 Community Health Improvement Grant by Dignity Health Arizona
Bridges Reentry has been awarded a second year of funding through CommonSpirit Health's Community Health Improvement Grants Program.
At a recent recognition event held at Creighton University, Bridges Reentry received a $100,000 grant award to continue expanding our implementation of the Transition to Success (TTS) framework. This investment builds upon last year's $119,000 grant and reflects CommonSpirit's ongoing commitment to improving health outcomes and quality of life for vulnerable and underserved populations across Arizona.
CommonSpirit Health's community investment strategy recognizes that health extends far beyond medical care. By supporting organizations that address housing stability, economic opportunity, education, and social connection, CommonSpirit is helping communities tackle the social drivers of health that significantly impact long-term well-being.
This new funding will allow Bridges Reentry to deepen and expand our work with women impacted by incarceration. Specifically, the grant will support:
Continued implementation of the Transition to Success (TTS) framework across our programs
A new in-prison TTS pilot serving 10-12 incarcerated women prior to their release
Ongoing support and outcome tracking for approximately 40 women participating in Bridges Reentry programs
Continued focus on economic mobility, goal achievement, and long-term self-sufficiency
The investment comes as Bridges Reentry continues to demonstrate strong outcomes for women navigating the transition from incarceration to independence. Recent program evaluations show meaningful improvements in critical areas including housing stability, employment readiness, financial well-being, and personal goal attainment.
These results reinforce what we witness every day: when women are equipped with the tools, support, and mindset to envision and pursue a different future, lasting transformation is possible.
We are grateful to CommonSpirit Health and Dignity Health Arizona for their confidence in our mission and for recognizing the importance of investing in women who are rebuilding their lives. Their partnership helps make it possible for more women to achieve stability, reunite with family, secure employment, and create brighter futures for themselves and their children.
Thank you to our donors, volunteers, mentors, staff, and community partners who make this work possible. Together, we are proving that second chances change lives.
